Temporary Change of Transportation Form

• If you have a TEMPORARY change of transportation, please send this form with your child to school, or bring it to the school office. For your child’s safety, we cannot take changes of transportation via email or phone.

Permanent Change of Transportation Form

• If you have a PERMANENT Change of Transportation, please send this form with your child to school, or bring it to the school office. For your child’s safety, we cannot take changes of transportation via email or phone.

REMINDERS:  

• Students may only ride their assigned bus.
• Early Dismissals AFTER 1:30 PM are discouraged to ensure the safety of our students.
• Anyone picking up a student must be on the child’s emergency contact list, must be 18 years of age or older to pick up from school, and must present a driver’s license.
